One day my nurse Mary Sanders, when getting a patients's weight (an Orthopaedic vital sign) asked the patient "How much do you weigh? (You know ball park to set the old scales to an approximate level) The response "235 lbs." Mary thought, (fortunately didn't say) "No way!" The actual weight-325 lbs!!! This was a case of number dyslexia or the worst case of denial we had seen in a while!

Some Dude If you have ever worked in an emergency room, you certainly have encountered this. When asking what happened to the guy with the gunshot, knifing, or physical assault, the response will be.  “Some dude just came up and whacked me.  I wasn’t doin nuthin‘. He just came up, out of the blue, and he whacked me” “Some dude” is everywhere. Every city and county. He comes up, unprovoked, and just whacks innocent people just “minding my own business” And he has brothers!  Big Dude, Little Dude Bad Dude  and the worst of all Mean MF Dude.  There is a family of them. If law enforcement was serious about cutting down violent crime, they would arrest “some dude” and lock him up.

More Bad Prognositc Signs Enablers-very bad sign  There are more people in the room than there are chairs You ask the patient a question, one they are perfectly capable of answering, and the “enabler” answers it for them. Especially the ones that the patient is the only one who can answer it like “What is the pain like” and “how bad is it?” The Shoulder Rubber”. The worst. As you are talking to the patient, they are rubbing the patients shoulder and telling them”It will all be alright” “What is the matter with these?” you ask. They are just providing support . The problem is the enablers take over and the patient takes no responsibility for themselves. Chances of improving go way down if there is not buyin from the patient themselves

I'm going to post an intermittent series of "Bad Prognostic Signs." Those in medicine know exactly what these are. For those who are not-they are things patients said and did, things you observed that indicate the future is not going to go well! The patient threatens you “I’m going to give you a bad patient satisfaction survey” “I’m going to sue you” “You’re really going to be sorry if you don’t give me those pain pills” More to come!
